Application for Enrolment

The following information is relevant for enrolments:

All families are required to complete an Application for Enrolment, which is reviewed by the School Principal. Please provide the following items with the Application and email all documents to northbeach.ps@education.wa.edu.au

  • Completed Application for Enrolment (attached below)
  • Birth certificate or passport
  • Medicare ACIR (Immunisation record found at www.my.gov.au)
  • Two forms of Proof of address (i.e. current utility bill and rates notice/contract of sale or tenancy agreement)
  • Visa (if applicable)
  • Information relating to health or medical condition, disability or additional needs (if applicable)
